Many noteworthy advances in our knowledge of the pericardium, its functions and diseases and their relation to heart failure have been made since the first edition of this book appeared in 1981; and no other book that covers in detail the physiology and pathophysiology has since been published. The first edition was favourably received, and I have frequently been asked to write a new edition. My own knowledge in the years that have passed since then, and my clinical and research experience in the field of the subject have both increased. For all these reasons, I decided that the second edition was overdue. The long time that has elapsed between editions necessitated rewriting, rather than simply revising, most of the text. For the same reason, many of the figures are new. Most of the references I have cited appeared in the literature after 1981, but I have retained a number of earlier ones, either because they are classics or, in my opinion, have not yet been bettered. It is my hope that the new edition will be a useful resource for clinicians called upon to manage patients with pericardial disease and for physiologists when the pericardium is relevant to their investigations. I make no apology for the in-depth treatment of the pericardial physiology and pathophysiology throughout the book, for they are the foundation on which diagnosis, hemodynamic and imaging studies, and management must rest.

This book will go a long way toward redressing the widespread ignorance of the clinical and functional significance of the pericardium. The author, a master physiologist-cum-clinician in the mold of Louis Katz and William Dock, has produced a treasure trove of facts and personal insights, with many historical excursions and interpolations, to illuminate the state of the art and the state of the science. In keeping with his personal interests, the book is asymmetric: 62 pages (almost one fifth of the text) cover constrictive pericarditis; chapter 2, "Physiology," covers 54 pages; pulsus paradoxus gets 28 pages. Most of the text on common forms of acute pericarditis is highly concentrated at the end of the book; the author dismisses inclusive diagnostic lists as being of more interest to the epidemiologist than to the practitioner. Unfortunately, an overwhelming number of patients qualify for a diagnosis of acute pericarditis under "idiopathic" rubrics, which undoubtedly mask cases of uncommon causes that would not come to mind without the aid of checklists. (For example, pericarditis occurs in more than 30 individual entities in the vasculitis-connective-tissue disease group alone.) (Figure) Some of the most fascinating reading concerns the author's personal experiences in the experimental laboratory, where he elucidated conditions that permit pulsus paradoxus, and in a fascinating section, "Comparative Physiology," that describes his work on the pericardium of the elasmobranch fish, which, despite the presence of a pericardioperitoneal canal, can support tamponade. Equally interesting is the analysis of pericardial pressure as measured by the tip of a catheter, versus the nonuniform, pericardial contact force as measured by flat balloon, and the consequences of the former procedure for cardiac physiology, especially preload and true cardiac filling (i.e., distending, or transmural) pressures. Chapter 3 is an almost chatty practical guide to the identification, almost exclusively by echocardiography, of the presence and management of pericardial effusion. The 12-page section on techniques for imaging pericardial effusions is outstanding. Chapter 5, "Pericardiocentesis," comprises 21 pages that are packed with practical how-to information and illuminated by the author's personal insights; this chapter also describes pericardioscopy and balloon pericardiotomy. Elsewhere, devices for myocardial support are mentioned, principally the acorn jacket, which is designed to act like the pericardium -- it is more compliant in the long axis than in the short axis -- to support the myocardium during cardiac failure. There is also a concise discussion of the uses of pericardial tissue for the construction of bioprostheses. The book has a few weaknesses, although certainly none in its factual material, its functional analyses, or its literary standards. The categorical title, The Pericardium, seems to promise comprehensive, if not encyclopedic, coverage. (The author may know that certain panjandrums of cardiology inveigh against "encyclopedic" inclusiveness.) Yet, except for fluid components, there is surprisingly little mention of the rich microphysiology of the pericardium. It is a veritable biochemical factory integrated with a complex autonomic-nerve supply, and this makes it particularly strange to some of us that so many patients seem unharmed by pericardiectomy. Perhaps such patients are either not followed long enough or not followed for the critical information. The index of the book is relatively anemic, with some topics not included even though they are well covered in the text, and a few topics, such as Dressler's syndrome, are indexed to the wrong pages. These drawbacks, even the near-absence of the subject of pericardial microphysiology, pale beside the excellence of the areas that are covered and the particular asset of this text: the author writes so well that one wishes the book were longer and included subjects that have been omitted or only lightly touched on. For example, one cannot improve on Shabetai's description of the pericardium as "beautifully designed for the normal heart and [for] cop[ing] nicely with early stages of chronic heart failure. In severe or acute heart failure, the pericardium is an obstacle to maintaining a normal or compensated hemodynamic status." The high standard of narration, as expected of a Briton (Shabetai is from Manchester), makes almost every page a delightful read. Many physicians, particularly cardiologists and physiologists, who pick up the book for spot reading are sure to acquire one or more copies. The Metaphysics of Gender is a book about gender essentialism: What it is and why it might be true. It opens with the question: What is gender essentialism? For example, the form or essence of an artifact, like a house, is what unifies the material parts of the house into a new individual (over and above a sum of parts). Since an individual's gender is a social role (or set of social norms), the kind of unity in question is not the unity of material parts, as it is in the artifact example. Instead, the central claim of gender uniessentialism is that an individual's gender provides that individual with a principle of normative unity-a principle that orders and organizes all of that individual's other social roles. An important ingredient in gender uniessentialism concerns exactly which individuals are at issue-human organisms, persons, or social individuals? The Metaphysics of Gender argues that a social individual's gender is uniessential to it. Gender uniessentialism expresses the centrality of gender in our lived experiences and explores the social normativity of gender in a way that is useful for feminist theory and politics.

Interesting Enough

This work did not contain the clearest metaphysical analysis and has some metaphysically uncomfortable consequences. But it's an interesting alternative to a materialist conception of gender and employs some concepts that are useful in understanding some gender phenomena, if not the nature of gender itself.

The book consists of 19 chapters making up 8 separate parts. The chapters in each part focus on related aspects of institutional pharmacy practice. The format provides useful tools. Throughout the book, key points and their significance are offset from the narrative to highlight their importance. All chapters conclude with a summary as well as suggested additional reading and references. A good discussion is provided on the categories of infusion therapies, various delivery systems for parenteral drug therapy, and methods of parenteral administration. Risks of parenteral therapy are alsoa focus of this section. Managing People is the seventh section in the text. Several definitions for leadership are considered, pointing out the difference between leaders and managers. Theories on leadership and styles of leadership are presented. Some personnel issues targeted in this section include recruitment, retention, motivation, and termination. The importance of planning a career path while still in pharmacy school is stressed with details given on advanced training opportunities for pharmacists. About the Author
Dr. Holdford is Associate Professor & Vice Chair for Graduate Studies, Department of Pharmacy at the Virginia Commonwealth University School of Pharmacy, Richmond, Virginia. He completed his Doctoral Degree at the University of South Carolina where he specialized in pharmaceutical marketing and Pharmacoeconomics. Prior to joining the faculty at Virginia Commonwealth University (VCU) in 1995, Dr. Holdford completed his BS in Pharmacy at University of Illinois in Chicago and an MS in Pharmacy Administration at Ohio State. He worked as a pharmacist and manager at hospitals in Chicago and Columbia, South Carolina.

At VCU, Dr. Holdford conducts research, teaches, and consults in the areas of pharmaceutical marketing, health outcomes research, and Pharmacoeconomics. He is also the author of the published textbook Marketing for Pharmacists (2007) and co-edited/authored the book Leadership and Advocacy for Pharmacy (2007). He has authored over 50 papers and book chapters in health care and business publications.




Dr. Brown was born in Jackson, MS in 1940, completed High School at Forest Hill, his BS in Pharmacy in 1963 and MS in Hospital Pharmacy in 1971 at the University of Mississippi and Doctor of Pharmacy in 1974 at the University of Tennessee. His employment includes several years in community practice, two years with Eli Lilly and Company in sales, four years in the US Army Medical Service Corps including duty as a Company Commander, 3d Infantry Division Medical Supply Officer and two years as Director of Pharmacy at the US Army Hospital, Vicenza, Italy. He joined the faculty at Ole Miss in 1970 became full Professor in 1985 and served as Chair of the Department of Pharmacy Administration for seven years (1986-1992). Dr. Brown has published numerous professional articles, four editions of the Handbook of Institutional Pharmacy Practice as Editor or Co-Editor with Dr. Mickey Smith, is a member of numerous professional, honorary and leadership organizations, and has received several awards including being named a Fellow in the American Society of Health-System Pharmacists. Dr. Brown retired from full-time teaching in 1999 but has continued to teach part-time. He and his wife, Bonnie, reside in Oxford, MS and have two sons (a pharmacist and a physician) and three grandchildren.

About the Author
Richard L. Daft, Ph.D., is the Brownlee O. Currey, Jr. Professor of Management and Principal Senior Lecturer in the Owen Graduate School of Management at Vanderbilt University, where he specializes in the study of leadership and organization theory. Dr. Daft is a fellow of the Academy of Management and has served on the editorial boards of Academy of Management Journal, Administrative Science Quarterly, and Journal of Management Education. He was associate editor-in-chief of Organization Science and associate editor of Administrative Science Quarterly. Dr. Daft has authored or co-authored 13 books, including The Executive and the Elephant, Organization Theory and Design, and Management. He also has authored dozens of scholarly articles, papers, and chapters, and he has published in the Administrative Science Quarterly, Academy of Management Journal, Academy of Management Review, Strategic Management Journal, Journal of Management; Accounting, Organizations and Society, Management Science, MIS Quarterly, and Organizational Behavior Teaching Review. Dr. Daft has received several government research grants in organization design, organizational innovation and change, strategy implementation, and organizational information processing. An active teacher and respected consultant, Dr. Daft has served as associate dean and helped manage a start-up enterprise. He has been involved in management development and consulting for numerous organizations, including the American Banking Association, AutoZone, Bridgestone, Vulcan Materials, TVA, Pratt & Whitney, Allstate Insurance, State Farm Insurance, the United States Air Force, the U.S. Army, Central Parking System, USAA, Bristol-Myers Squibb, and Vanderbilt University Medical Center.

Dorothy Marcic, EdD, MPH, is a professor at Columbia University and a former faculty member at Vanderbilt University. Dr. Marcic is also a former Fulbright Scholar at the University of Economics in Prague and the Czech Management Center, where she taught courses and did research in leadership, organizational behavior, and cross-cultural management. She teaches courses at the Monterrey Institute of International Studies and at the University of Economics in Prague, and she has taught courses and given presentations at the Helsinki School of Economics, Slovenia Management Center, College of Trade in Bulgaria, City University of Slovakia, Landegg Institute in Switzerland, the Swedish Management Association, Technion University in Israel, and the London School of Economics. She has served on the boards of the Organizational Teaching Society, the Health Administration Section of the American Public Health Association, and the Journal of Applied Business Research. Dr. Marcic has authored 12 books, including Organizational Behavior: Experiences and Cases, (South-Western Publishing, 6th Edition, 2001), Management International (West Publishing, 1984), Women and Men in Organizations (George Washington University, 1984), and Managing with the Wisdom of Love: Uncovering the Virtue in People and Organizations (Jossey-Bass, 1997), which was rated one of the top 10 business books of 1997 by Management General. In addition, she has had dozens of articles printed in publications such as Journal of Management Development, International Quarterly of Community Health Education, Psychological Reports, and Executive Development. In effect, although we have many publications that deal competently with particular periods or systematically with different topics of its past, a comprehensive history of Vietnam is still lacking. That is the reason I am happy and humbled to introduce here A Story of Vietnam.

A Story of Vietnam treats evenly all the periods and also gives equal importance to the culture and the arts as to the political or military events of Vietnam's past. I call it a story and not a history, because I do not want my book to be the usual conventional textbook, overburdened with interminable academic, historical and bibliographic references.

While not a conventional textbook, A Story of Vietnam can, nonetheless, provide a substantial reading material to students interested in Asia. To the hyphenated Vietnamese, it can serve as a convenient reference tool to the historical allusions, cultural insinuations, mythical hints, literary suggestions, ethnic idiosyncrasies they encounter every day at home. This book may also be sought after by the people who know so much already about Vietnam as a War but who still would like to know more about Vietnam as a culture.

I have narrated my story with the greatest impartiality I am capable of. I have no theory that needs to be proven nor do I have any assumption to be verified. But I do come to history with emotion, even with passion. Sometimes, my sympathies surged to the surface or my distastes became apparent, though at no time, have I consciously distorted the facts or altered the documents in order to validate my feelings.

The ten chapters of this book are naturally of unequal length. They adhere strictly to the chronological order, meaning that Chapter One deals, among others, with the legendary origins of the Vietnamese people and the last chapter, Chapter Ten, recounts the social traumas, the economic hardships, and the political isolation the country experienced after reunification in 1975 to the remarkable recovery effected since 1986 and culminating in October of 2007 when Vietnam was elected as a non-permanent member of the Security Council of the United Nations.

Truong Buu Lam is a retired professor of History from the University of Hawaii.

As I wrote this book, I always kept in mind the generations of hyphenated Vietnamese who have had no opportunity to learn much about the land of their ancestors in schools of their new country. I wanted to narrate to them the story of Vietnam from the beginning to the present. I also meant to relate the political events of Vietnam's past to its cultural features so that my readers can experience Vietnamese society in its wholeness.
I also wish this work will find its way to the people who know already so much about Vietnam as a war but who would still like to read something about Vietnam as a people, a country, a culture.

It has now become an incontestable fact that the USA has failed to attain the twin goal it had assigned to itself in Vietnam. Besides the failure to realize its objectives, did the US suffer a military defeat in Vietnam?

There are more and more people who would answer no to that question, arguing that the USA did not go into that war with the will to win. It is quite difficult for me to understand such an argument, for does it not represent a very serious indictment of the American establishment? How could any government order its troops to shoot and kill hundreds of thousands of persons, rain more than four million tons of explosives and seventy two million liters of chemicals all over the territory of another country that is thousands of miles away from the US and to a people that has never done any harm to the United States?

Other scholars blame the US government for having bound the hands of its strategists by making the decision to fight only a limited war. Can the Vietnam War be seriously called a "limited war" when, at its peak, more than half a million of American armed persons had to be ferried half way around the globe to a country that is about the size of California? One might ask what would the generals have done had their hands not been tied down? Would they have used tactical nuclear bombs? Would they have bombed Vietnam back to the Stone Age? Would they have created a desert out of Vietnam and call it an American Victory or put on that desert the label Pax Americana?

A Story of Viet Nam

Even though I am over 60 and had lived 30+ years in Viet Nam, I found myself learning as I read A Story of Viet Nam. The author, Professor Truong Buu Lam, meticulously and comprehensively narrated the history, culture and people of Viet Nam. The book is worth reading, especially for those with a curiosity to explore Viet Nam.

I don't usually review books (or anything else for that matter), but I read "A Story of Vietnam" and then saw the negative review of CrtclRdr. I can not help but have to put in my 2-cents:

As a Vietnamese-American, growing up during the war, I read this book and think that it is extremely well done. Please note that the book's title is "A Story of Vietnam" not "A History of Vietnam" (I suffered through History classes with all these endless names and dates, so I would never have been interested in this book if it were supposed to be a fact-laden tome of history!). The author is not trying to produce a documentary but a readable "STORY" about Vietnam, so "proper citation and alternative point of views" is really not the point here. He has done an excellent job at weaving together the cultural, historical, literature, artistic, social and political background to tell the story of Vietnam in the most neutral way possible, taking in the point of views from many sides.

As a person telling a story, Dr Truong is entitled to come up with interpretations of his own, which I think is very refreshing and interesting. However, no where in the book I found "deliberate distortion of historical and archaeological finding", or "misinterpretation of historical and cultural references". Don't get me wrong, I am not an expert on these subjects so I am not really qualified to labeled anything as "biased" or, "misinterpreted", or "misrepresentation" or "distorted", or otherwise. I am just a Vietnamese who have lived almost half of my life in Vietnam, with first-hand experience of the Vietnamese culture. I know the history of Vietnam as it was being told and re-told from both sides. From that background, I found the book very informative as well as enjoyable, and did learn a lot about the factual happenings that took place during the war.

As a trained scientist, I also know that cultural and archaeological findings are open to interpretations. Scientific data may substantiated or refute an interpretation, but "interpretation" is not considered an exact science. So, "in the context of historical and cultural information", a "proper citation" is just a reference to another person's interpretation without the guarantee that it's the truth. An interpretation is just that, not a proven fact. You can agree or disagree, but you can not say that it is correct or incorrect. As an archeological researcher and a professor of History, I think Dr. Truong is well qualified to have his own interpretation. I have an impression that CrtclRdr is a well known expert on the history and culture of Viet Nam, who knows more than enough about the subject to challenge the credibility of the author, but "CrtclRdr" must not be his/her real name, otherwise, I could have recognized it...

I gave this book 4 stars because I agree with another reviewer about the production quality. The typos and miss-prints distracts. I really don't know why this reader is so unhappy with the book, but my 2-cent is: DO "waste" your money, buy this book, read it, and find out for yourself...

Insightful book from a Vietnamese scholar perspective

Being a Vietnamese-American growing up in the US, I wanted to know Vietnamese history that spans over the history of the nation rather than just focusing on the history around the Vietnam war. I was happy that this book accomplished that goal. I like the peppering of legends and folk tales in between historical accounts. Having heard some of the stories and bits of history from my parents, this book puts things in perspective. I like the fact that the author acknowledge that it's a "story of Vietnam" rather than the history of Vietnam. He made it clear where the source of the various of the accounts come from so we can judge its accuracy ourselves. Dr. Truong was balanced in providing viewpoints from the local Vietnamese, the Chinese, and the French. After reading the book, I bought several copies as gifts to my siblings and friends. They, too, enjoyed the read. Although some finds the interjection of folk tales and legends in between historical accounts to be distracting and confusing. Overall, I highly recommend this book to people who wants to know a more in depth story of Vietnam from a non-western perspective.

Great value series, very enjoyable read...

Krinos (Take Over Book 1)

Krinos is an interesting story which follows Eliza or as most people know her Krinos who's being prepared by her father to take over the family drug business. She's a feisty alpha female if there is such a thing and she's prepared to do whatever it takes. She meets hot as hell Stefanos, who for the most part rocks her world. Stefanos also becomes very smitten, very quickly with the bad ass Krinos. Krinos is her father's daughter and she has no qualms about the fact that she will become boss of the "business”.

This story contains violence, drugs and sex. It is an interesting story and is a nice quick read. It’s only Book One of the Take Over Series by T.L. Smith but does not end on a cliff hanger although there is more to come I am sure. I read this book pretty much in one sitting and found it a great read. I gave this book 5 stars.

Kalon (Take Over Book 2)

Kalon, is the second book in the Take Over series. It continues where the first book finished and is the story of Eliza (Krinos) and Stefanos, her Greek God.

Krinos, is a tough woman, very tough and she really shines through in this portion of the story. We are introduced to a number of new characters, which all add to the intrigue of the story line. It also shows a softer side to Krinos, although she would hate for me to say that.

Who wouldn't love Stefanos? He really is a Greek God in all ways. He's definitely one of my current book boyfriends. Just love his dominant and demanding ways.

This is a very quick read, but I loved it. I absolutely cannot wait for the third installment. I give this book five stars.

Kratos (Take Over Book 3)

Kratos is written in Stefanos’ point of view and most definitely does not disappoint. This story just gets better and better and this book completes the Take Over Series. It gives a little background of the young Stefanos and then continues on from book two.

I love Stefanos’ point of view, it shows how he changes into the man he is, once he meets Eliza or his “Princess”. Book three takes on a different path when finally Stefanos becomes Eliza’s Kratos.

Although this is a short novella, it packs a punch and I thoroughly recommend the final book. I give this book a five star rating and cannot wait for more books by T. L. Smith.

I enjoyed the feelings of possessiveness he characters portrayed throughout the book. So sexy and intense! A fun read that kept me wanting to know about the other minor characters.
